My Way Community Alliance is a friendly, welcoming and socially responsible community organisation. We provide NDIS, Disability, youth and elderly support services. Our vision and mission is to support meaningful, valued and contributing roles for all Australians.
The Community Service Coordinator will work closely with NDIS participants with disability, psychosocial disability, their families/carers/guardian and support workers to deliver quality, person centred, and goal focused outcomes in an efficient and safe working environment.
The roles involves:
· Coordinating My Way services by meeting the individual needs of our participants through:
· Liaison with formal and informal support networks including Support Coordinators, LACs, Family, Carers and Guardian
· Onboarding of NDIS Participants into the Client Management System - MYP
· Completion of risk assessment and ensure it is reviewed at least annually or earlier in collaboration with each participant, according to their changing needs or circumstances
· Development of Support Plan that reflects participant needs, requirements, preferences, strengths and goals, and is reviewed at least annually or earlier in collaboration with each participant, according to their changing needs or circumstances
· Tracking the progress of participant’s goals by ensuring our services are working towards achieving participant’s goals as per NDIS plan
· Matching the right Support Worker to participant as per their preferences
· Developing and reviewing roster templates for each participant and ensuring the roster schedule is published up to a month for Support Workers
· Approving Support Worker timesheets when shifts are completed
· Monitoring of participant’s NDIS budgets and NDIS Plan dates if it has been reviewed or extended on Client Management System
· Act as a point of contact for participants and support workers in the coordination of My Way services, including resolving points of crisis
· Report writing within the agreed reporting frequency to NDIA, on outcomes and recommendations in the provision of our Community Support services
· Build the capacity of the participant to achieve greater independence to self-direct services and supports in the longer term
· Ensure support workers are competent and confident to support customers with complex health needs including manual handling techniques, medication competency and any other associated training required
· Build a positive team environment through the demonstration of good communication and interpersonal skills through peer support
· Model appropriate conflict resolution, communication, and positive behaviour management skills
· Plan and facilitate regular and effective team meetings and professional development opportunities with direct Support Workers
· Maintain a safe and healthy workplace for the team and customers
Work collaboratively with the NDIS Service Manager to contribute to the environment of continuous improvement in the area of Service Coordination
Required Knowledge, Skills & Experience
· Relevant tertiary qualification such as Master of Community Development
· Relevant qualification in the Disability Sector, such as Certificate IV in Disability or Certificate III in Individual Support
· Relevant leadership experience in sector, such as experience as a House Coordinator.

Pay: $40.00 per hour
