Salary range: $54.44 to $58.30 p.h. (Nurse Grade 6 Band 1)
4 to 6 weeks annual leave with 14%-17.5% loading
12.75% employer contribution to superannuation
Plus Salary Sacrificing options may be available

About your opportunity

This role involves comprehensive discharge planning, including assessing individual needs, coordinating with multidisciplinary teams, educating families, arranging necessary medical equipment, and establishing follow-up care.

The discharge coordinator is responsible for scheduling and monitoring weekly Retinopathy of Prematurity (ROP) checks with an ophthalmologist for at-risk infants, as well as organizing and attending developmental follow-up clinics to track growth, neurological development, and initiate early interventions when needed.

Requirements:
Qualification: Bachelor of Nursing or equivalent certification relevant to the position is mandatory.
Highly Desirable: Possession of a relevant postgraduate qualification in an appropriate field is highly desirable.
Attainment of Neonatal Specific Competencies are mandatory for all nursing staff working within the Neonatal Unit.
Shift Workers: Appointees will be required to work all shifts in accordance with the unit roster.
At least 5 years recent clinical experience in a Neonatal Unit.
