Care to Heal is a growing NDIS provider committed to empowering people with disabilities to live full, meaningful, and self-directed lives. We’re now seeking a compassionate and organised NDIS Support Coordinator to join our team and help participants make the most of their NDIS plans.

What You’ll Be Doing
As a Support Coordinator at Care to Heal, you will:
- Assist participants in understanding and implementing their NDIS plans
- Assessing clients ' and maintaining the appropriate paperwork to facilitate their needs
- Coordinate a range of supports and services across health, housing, community, and mainstream systems
- Work closely with participants, their families, and providers to build capacity and achieve goals
- Maintain accurate case notes, service records, and reports in line with NDIS requirements
- Identify and respond to risks, and help participants navigate any barriers to accessing services
- Support participants through plan reviews, changes in circumstances, and reassessments
- Promote choice and control by empowering participants to make informed decisions
- Liaise regularly with internal teams and external providers to ensure continuity of care
We’re Looking for Someone Who:
- Has experience in a support coordination, case management, or NDIS-related role
- Has strong knowledge of the NDIS and its funding models
- Demonstrates excellent communication, organisation, and problem-solving skills
- Can manage competing priorities and work independently
- Has a person-centred, strengths-based approach to supporting others
- Understands the importance of documentation, confidentiality, and professional boundaries
- Is passionate about making a positive impact in the lives of people with disabilities
Requirements
- Minimum Cert IV in Disability, Community Services, or equivalent (higher qualifications desirable)
- Experience working with NDIS participants or within the disability/community sector
- Working with Children Check (WWCC)
- NDIS Worker Screening Check
- Current Australian Driver’s Licence
- COVID-19 vaccination (or valid exemption)

Job Type: Full-time
Pay: $75,000.00 – $90,000.00 per year
Schedule:
- 8 hour shift
- Monday to Friday
- On call
- Overtime
- Weekend availability
Supplementary Pay:
- Overtime pay
