About the role
Northern NSW Local Health District has a fantastic opportunity for an experienced and passionate nurse to contribute their expertise in a supportive and dynamic environment and join our dedicated teams either at Lismore or Grafton Community Health as a Nurse Practitioners (NP) or Transitional Nurse Practitioners (TNP). This role enhances access to healthcare for "Older Persons'" by addressing service gaps within dynamic Models of Care (MoC). Improving the health of older persons by providing timely access to specialist aged healthcare, preventing deterioration, reducing hospital presentations and admissions, while collaborating with other healthcare team members.
The Transitional Nurse Practitioner is appointed for a provisional period of up to 3 years to work towards endorsement as a Nurse Practitioner.
