About the Business Unit:
HM Prison Barwon is a maximum-security prison located 6km north-west of Lara, 20km from the centre of Geelong and about 70km from Melbourne, and is part of the Barwon South West Regional structure. It is Victoria's major maximum-security prison for sentenced male prisoners. The prison officially opened in January 1990 and currently houses up to 416 men in custody, including high security, long term management, protection (including specialist protection), general mainstream (maximum security) and medium security prisoners.
About the Role:
The Specialised Lead Assessment Transition Coordinator forms part of the multi-disciplinary Vocational Services Team, which is responsible for the development, co-ordination and delivery of a range of programs and services across several specialist areas.
Some of your duties will include:
- Delivering the CV Reintegration Pathway to men with complex care and high transitional needs, quite often located in restricted regimes
- Establishing and maintaining local processes in conjunction with key stakeholders to ensure coordinated discharge planning with a focus on facilitating prisoner engagement in mainstream services on release
- Participating in multidisciplinary meetings such as case planning and care team meetings.

About You:
As our ideal candidate, you will possess:
- Demonstrated detailed knowledge of specific transition and reintegration issues and needs of prisoners with complex and high needs
- Demonstrated detailed understanding of the CV Reintegration Pathway
- Demonstrated ability to administer risk and other assessment tools with individual high needs and complex prisoners.
Qualifications/Requirements:
- Expertise in conducting of varying assessments, programs and service delivery/coordination within a correctional environment is highly desirable.
