We are seeking a motivated and compassionate Client Relationship Officer to join our Melbourne team. In this pivotal role, you’ll be the key connection between our participants, their families, our support workers, and external stakeholders—ensuring the highest standard of care and service delivery.
Key Responsibilities
- Provide direct assistance to NDIS participants and their families, understanding their unique needs.
- Develop and maintain strong networks with referrers and community stakeholders.
- Create and manage individualised support plans that reflect each participant’s goals.
- Conduct client risk assessments to ensure safety for all.
- Monitor case notes, follow up promptly, and ensure accurate reporting.
- Collaborate with support workers, clients, and stakeholders on incident reporting in line with NDIS requirements.
- Promote NSW Disability Care services as a brand ambassador.
- Support participants to build skills, achieve their goals, and actively engage in the community.
- Establish support budgets based on NDIS funding and plan periods.
- Provide timely progress reports to Support Coordinators when requested.
Qualifications & Requirements
- Tertiary qualification in Mental Health, Nursing, Disability, Social Work, or another relevant human services field.
- Sound knowledge of the NDIS, especially in relation to client services management and participant choice and control.
- Proven ability to develop positive, collaborative relationships with participants, families, and service providers.
- Excellent verbal and written communication skills, with the ability to prepare clear reports and correspondence.
- Strong interpersonal skills with cultural sensitivity and understanding of diversity.
- Practical problem-solving skills, with a proactive and professional approach.
- Current Australian driver’s license, with the ability to travel to meet clients at their residence or in the community

Job Type: Full-time
Pay: $37.00 – $39.00 per hour
Expected hours: 38 per week
Experience:
- working in the NDIS sector: 1 year (Preferred)
Licence/Certification:
- Working with Children Check (Preferred)
- NDIS Screening Check (Preferred)
- Driver Licence (Preferred)
