About the Role:
Worthy Ability is seeking a compassionate and dedicated Key Worker to join our team. As a Key Worker, you will play a crucial role in supporting individuals with disabilities, helping them achieve their goals, and enhance their quality of life through person-centered care. This role offers the opportunity to make a significant impact on the lives of individuals and their families.
In this role, you will work closely with clients, their families, and a multidisciplinary team to ensure high-quality support services are delivered in alignment with the NDIS framework.
Key Responsibilities:
- Client Support & Advocacy: Serve as the primary point of contact for clients, providing guidance and support in all aspects of their care and advocating for their needs.
- Care Coordination: Develop and implement tailored care plans that align with each client’s goals and NDIS funding requirements.
- Family & Stakeholder Engagement: Work closely with families, caregivers, and other professionals to ensure consistent, holistic support for clients.
- Progress Monitoring & Reporting: Track client progress, make adjustments to care plans as necessary, and ensure accurate documentation in line with NDIS standards.
- Training & Support: Provide training and support to families, carers, and team members in implementing behavior support strategies and other necessary interventions.
Essential Skills & Qualifications:
- Relevant Qualifications: A degree in Social Work, Psychology, Occupational Therapy, or a related field.
- Experience: Experience working with individuals with disabilities, ideally within the NDIS framework.
- Communication Skills: Strong interpersonal skills and the ability to engage effectively with clients, families, and team members.
- Knowledge of NDIS: Familiarity with NDIS policies, procedures, and individualized care approaches.
- Problem-Solving Abilities: Capacity to develop innovative solutions to support individuals in achieving their goals.
- Team Player: Collaborative attitude and ability to work in a multidisciplinary team.
