Role Overview:
We are seeking an experienced NDIS Support Coordinator to join our team in Western Sydney. In this role, you will assist NDIS participants to implement their plans, connect with service providers, and achieve their goals. You will provide ongoing support to ensure services are delivered effectively and meet the individual needs of each participant.
Key Responsibilities:
- Manage a portfolio of NDIS participants, connecting them with appropriate service providers.
- Monitor plan expenditure and support participants to make informed decisions about their services.
- Assist participants with navigating the NDIS portals and selecting providers aligned with their needs.
- Regularly review participant progress and report outcomes to the NDIA within required timeframes.
- Ensure compliance with the NDIS Practice Standards and relevant legislation, including the provision of reasonable and necessary supports.
Qualifications and Experience:
- Minimum Certificate IV in Disability Services, Community Services, or a related field, or equivalent experience.
- At least 2 years of experience in a support coordination role or a similar position within the NDIS or disability services sector.
- Strong understanding of the NDIS framework, including the NDIS Price Guide and reasonable and necessary supports.
- Experience in the implementation and management of NDIS plans.
- Excellent communication and interpersonal skills.
Required Certifications and Compliance:
- NDIS Worker Screening Check (or willingness to obtain).
- Completion of NDIS Quality and Safeguards Worker Orientation Training Module (or willingness to complete).
- Working with Children Check (if applicable).
- Current NSW Driver's License and a vehicle with third-party property insurance.
- Immunisation Status Clearance (including evidence of COVID-19 vaccinations or a valid medical contraindication certificate).
