Position Overview:
The NDIS Client Care Coordinator will act as the primary point of contact between clients, their families, and service providers. This role is responsible for coordinating and monitoring the delivery of services outlined in each client’s NDIS plan, ensuring they are implemented effectively and aligned with the client’s individual goals. The Coordinator will build and maintain strong, collaborative relationships with all stakeholders to support positive, person-centred outcomes.
Key Responsibilities:
- Manage client needs and goals through assessments and care planning.
- Ensure excellence in client service delivery, including goal planning and onboarding of new clients.
- Resolve client escalations and complaints in a timely and effective manner.
- Develop exceptional relationships with both existing and new clients and their families.
- Investigate and resolve incident reports, updating the incident report register.
- Update progress reports after each client visit and prepare progress reports for support coordinators.
- Liaise with support coordinators to provide feedback on client care.
- Regularly visit clients to assess well-being and ensure care quality.
- Provide client centered support and ensure the delivery of appropriate care.
- Develop and maintain routine processes.
- Monitor and approve payroll expenditure for employees.
- Participate in the recruitment process, including interviews.
- Be on call and fill in for workers when needed.
- Communicate, obtain and record necessary services information
- Ensure compliant documentation for clients as required by program auditors.
- Attending NDIS client review meetings when necessary.
- Providing support to administration staff as needed.
- Communicate with clients, employees and stakeholders by phone or in person.
- Contribute to planning and participate in organisational events and activities.
- Ensure systems are updated for accurate client reporting such as client details, support coordinator information, diagnosis in DSC.
Qualifications and Experience:
- Bachelor’s degree in social work, Psychology, Disability Studies, or a related field.
- Demonstrated experience in a similar role within the NDIS or disability services sector.
- Strong understanding of the NDIS framework and person-centered planning principles.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients, families, and stakeholders.
- Proven ability to develop and implement individualized support plans.
- Strong organizational and time management skills, with attention to detail.
- Proficiency in using client management systems and data reporting programs.
- Ability to work independently and as part of a multidisciplinary team.
- Current NSW driver's license and a vehicle with third-party property insurance.
Desirable:
- Knowledge of contemporary mental health carer issues.
Job Type: Full-time
Pay: $70,000.00 – $80,000.00 per year
