A) Position Description
JOB Title: Disability Support Worker.
JOB TYPE: Full-Time / Part-Time / Casual / Sub-Contractor.
LOCATION: (Northern Suburb VIC) Office and Client site / Victoria- Northern and Western suburbs
B) Qualifications (Documents needed)
1) NDIS Workers Orientation Program Module. (completed or willing to obtain)
2) Completed COVID-19 Infection Control Module. (completed or willing to obtain)
3) NDIS Workers Screening Check Or Current National Police Check. (current or willing to obtain)
4) A valid driver's license, access to a car and Car Insurance
5) Proof of Covid-19 Vaccine. (3 shots)
6) Working with Children Check. (completed or willing to obtain)
7) Australian Passport or Have Right to work in Australia. (Granted Visa) / PR / Citizenship Certificate / AU Birth Certificate.
8) A relevant qualification in Disability/Aged Care/Individual Support (Ex: Certificate III in Individual Support - Certificate IV in Ageing Support - Certificate IV in Disability). (Not Essential but would be advantageous)
9) First Aid certificate. (completed or willing to obtain)
10) CPR certificate. (completed or willing to obtain)
11) Asthma and afflicted. (completed or willing to obtain)
12) Health Care Card / MediCare. (if applicable)
13) Personal accident insurance or worker's compensation insurance (If Applicable)
14) Qualifications & Curriculum Vitae. (CV)
15) PEG Feeding (Highly Advanced – Only for one Position)
16) Manual Handling & Medication Certification (assistance with medication)
C) Competencies and experience
1) Previous experience in a similar role as a support worker – Minimum of 1-month relevant experience.
2) An understanding of the disability sector and person-centred support.
3) Professional or lived experience supporting someone with a disability.
4) Demonstrated proficient written, verbal and interpersonal communication skills.
5) Demonstrated ability to work with minimal supervision, and collaboratively as a member of a multidisciplinary team.
6) Ability to effectively prioritise and organise own work/caseload.
7) Demonstrated commitment to professional development, quality improvement and evidence-based practice.
8) Experience dealing with Complex Care requirements / Manual Handling / Food Handling / Infection Control
9) Flexible availability & a long-term commitment to CareMile.
D) Salary:-
1) Hourly rate (Weekday) $42 - $65 – Based on Experience.
2) Hourly rate (Weekend/Public Holiday) $50 - $65 – Based on Experience.
E) The role may involve:
1) Assistance with community access and recreational activities.
2) Personal care. (showering, toileting, and hygiene assistance)
3) Manual handling and the use of wheelchairs & hoists.
4) Transportation and social engagement.
5) Light domestic duties such as meal preparation and cleaning.
6) Behavioural and Mental Health Support. (Not essential but would be advantageous)
F) MAIN DUTIES / RESPONSIBILITIES
1) Support clients to participate in age-appropriate activities and tasks including employment, in-home support, and community access.
2) Support clients to access activities and outings in the community.
3) Provide direct assistance to each client in accordance with their service plan and as directed.
4) Work in accordance with the medication administration policy, work health and safety procedures and client rights policies including privacy and confidentiality.
5) Monitor the safety and well-being of the clients and report any concerns to the Manager.
6) Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required.
7) Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
8) Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
9) In conjunction with other members of staff assist in developing, implementing, and evaluating training and behaviour programs to attain identified individual goals.
10) Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
11) Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
12) Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.
13) Report regularly to the manager
14) Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
15) Provide backup and support to other staff, as necessary.
16) Participate in Performance Development meetings to identify ongoing training and support opportunities.
17) Notify the service if you are unable to work your rostered shifts, with as much notice as possible.
18) Contribute to Case reviews with Employment Consultants and Facilitators.
19) Take reasonable care of his or her own health and safety.
20) Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other persons.
21) Report any quality and safety incidents or hazards at work to their Manager or Supervisor.
22) Carry out their roles and responsibilities as detailed in the relevant policies and processes.
23) Keep themself fully informed of the contents of the NDIS system and apply it to daily work.
24) Flexibility to work at multiple locations and sites
25) Your role may involve providing assistance with all aspects of daily living including: meals, personal care, assistance with everyday household tasks, meaningful engagement within their home, engaging in the community (including but not limited to).
26) Experience assisting individuals with complex support needs.
27) Successful applicants will be flexible in their working hours, possess excellent communication skills and demonstrate personal attributes and qualities to provide exceptional support and care to people with disability individually and within a team.
