About the Role
As a SIL Case Worker, you'll be the heartbeat of our mission, providing unparalleled support and services to our participants under the National Disability Insurance Scheme (NDIS). Your role is crucial in building and maintaining positive relationships with participants, their families, and support networks. You will be the primary point of contact for participants, ensuring their needs are not just met but championed, within our organisation.
Key Responsibilities
- Develop and manage case plans tailored to individual participant needs.
- Engage actively with participants and stakeholders to enhance service delivery.
- Maintain meticulous records in compliance with NDIS guidelines.
- Advocate for participants' rights and empower them to participate in their care decisions.
- Stay informed on industry developments and participate in professional growth opportunities.
- Uphold our values of integrity, respect, equality, and community commitment in every interaction.
About You
- Passionate about equal standards of living and rights for all abilities.
- Proven experience in client-facing roles, preferably in health, social services, or related fields.
- Strong interpersonal and communication skills, capable of nurturing lasting relationships.
- A proactive problem solver who anticipates needs and delivers exceptional service.
- You are someone who displays critical thinking and a can do attitude in the face of challenge.
- Committed to personal and professional growth within a forward-thinking company.
- Knowledgeable about the NDIS framework and related legislation.
- Participate in a rotating after-hours roster.
