We're looking for multiple experienced Support Workers to provide continuous in-home care for an individual with high and complex needs at his own home situated in Caboolture. Candidates must be available for a 24/7 roster, including overnight awake shifts. Adherence to the support plan is essential. Our goal is to build a small, well-trained team dedicated to the ongoing care of our valued participant.
Specific training will be provided but it is essential candidates have experience in assisting previous NDIS clients who require intensive supports.
Additionally, you will:
- Provide personal care, mobility assistance, meal preparation, and household chores
- Support community access and social activities
- Aid with medication management and accompany clients to appointments
- Encourage independence and self-determination
- Promote community inclusion based on preferences and abilities
- Monitor changes and report promptly
- Maintain accurate daily shift reports and care records
About you:
- Minimum completed Cert III in Disability Support, Individual Support, or related field – completion certificate is a must
- Current First Aid Certificate and CPR Certificate or willingness to obtain before employment
- Demonstrated experience supporting individuals with disabilities
- Understanding of person-centred care and its practical application
- Familiarity with NDIS service delivery models
- Physical capability aligned with job requirements
- Strong communication skills for collaboration and team player mindset prioritising customer needs
- Empathy towards others' needs and proactive approach?
- Proven track record achieving customer outcomes within NDIS funding guidelines
- Proficiency in Microsoft Office Suite and computerised customer databases
- Hold an open class driver’s license, comprehensive car insurance policy certificate?of Currency, roadside assistance, and current vehicle registration?
- Availability to travel between clients daily for community work
- NDWS Clearance to Work from NDIS Worker Screening Unit & Blue Card when working with MS Queensland
- Completion of NDIS Online Worker Training Module required before starting the role
